Socket Adapter Fall Out Screwdriver

update 11/08/18 Right angle adapter has no problem described below. It fits securely into my driver. Milwaukee driver and falls out. I solved this problem by using a dermal diamond cutting disc to cut off about 3/16". Now it works correctly. I don't know yet if there is the same problem with a rectangular mount shaft on it as well.
