My dog is a 16 pound Rat Terrier mix that most people consider a medium sized dog. It has a 12 inch neck that puts it on top of the "small" and bottom on the "medium" collar. I chose the small one, and looking back, maybe that was a mistake. The collar fits perfectly. but the bow is too small for him. I wore it on Thanksgiving morning and my wife didn't even notice he was wearing a bow tie until she finally pointed it out to her as we were getting ready for bed! However, the old man (he's 17+) looked pretty dapper in it. The quality of the bow and collar is pretty good. No complaints in this area. However, there are a few subtle problems. First, the loop doesn't stay put. He spends more time under one of his ears on the side than in the middle of his neck. Secondly, the buckle and especially the D-ring bother. That means if you put his number and name tag on the D-ring, the D-ring will hang down the middle of his neck and a bow tie should be worn over the D-ring to compensate. but its nice. it is impossible. Secondly, using a leash also spoils everything. Luckily, the holidays are destined for lounging around the house, so I just let him wear the bow tie without overloading him with any extras. Incidentally, when the collars arrived, the first thing I tore off was the bells and threw them in the trash, so I can't say anything about the functionality. In general I am satisfied with the collars. I just want the bow tie to be more noticeable when he's wearing it.
