My size 8 wide foot fits, but these shoes are uncomfortable. Order by size?

I had high hopes for this shoe. And yesterday I wore it all day to school work. This made my big toes feel pinched. And my ankles felt worn down on the higher back of the shoe. I've thought about it a lot. I think if my foot was medium width they would be much better. If I had a medium foot I could add both extra panels to increase cushioning and lift my ankle off the top of the back of the shoe. This would reduce the wear and tear I felt on the back of the shoe. Anyway, I refused both insoles because they cramped my toes. - while my heels were still loose. A day passed and my thumbs felt like themselves again. If I had ordered a size up my toes might not have gotten caught on the end of the shoe and hurt. But then again, since the back of the boot doesn't hold my foot very well, it can still go forward to my toes. I get back to saying they just aren't wide enough for my toes due to the construction of the shoe. UPDATE: Above ankle height. If you add your own and/or any provided soles to these shoes, your ankle will move up in the shoe and possible ankle pressure will be moot.
