I have another SI belt that Revain sells that has a compression/support pad, but I've had issues with poorly designed rings that would tip over and make the belt longer and also dig into my skin. I bought this product because the reviews were good and I needed something to support my sacrum area when getting back to running after a hip and back injury over 2 years ago. It's much better made than my old one and the way the velcro adjusts on the sides is very secure. I give 5 stars if not for one problem: too big. I have a big one because I know my hips are over 40 inches. I lost a few pounds from my running efforts and now the belt is almost too big (although I'm pretty sure my hips are still 40"+). I even have an old support pad from my other belt tucked in the back and I'm sure it's taking up some space too. I would say that you should measure *tight* around your hips, not normal hip measurements. I sincerely wish this thing was too big for me because it would mean I was on my way to being active again, so maybe I could find a way to change it or give it to someone who needs it. The price for this is amazing. I was about to give up and buy a good Serola belt until I found this one. The only thing I would add is that it would be great to have an extra support/compression pad that comes with it. You can buy them separately but they can be expensive for what they are.
