Bought for a choir concert at the end of the year for my middle school student. He's 13, wears a 14 or 16 top and bottom. The size 16 shirt I bought fit just right, but probably won't fit next year. A size 14 would probably be too tight. He is 5 feet 4 inches tall and weighs about 110 pounds. So I'd say go up a size unless your child is too skinny, but definitely don't go down a size. The material is nice, easy to iron, the white almost shows through so make sure you get the shirt the right color. The collar was short, somewhere between the length of a tuxedo and the collar of a regular shirt, but it fit with our tie. I think it's a polyester blend. Well pressed, a few folds through the packaging, nothing major. Also bought suit trousers from the same brand in size 16 and a matching waistcoat from the same brand (size XL).
