Pad separated from backing

I glued this to a Fezzari Abaho front panel a week ago. Rode about 50 miles on a singletrack with significant descents. At the end of my last ride, it was determined that the squeaky noises were due to the pad material on one of the pads coming loose and the metal on one side of my disc was metal to metal. Even if I took the easy way out, I have to replace the drive. I've never had this problem. The other set on the back works fine.
