Medium size fits my very long and comfortably plump 16 lb cat. However, one day he didn't move. He just lays on his side and seems totally spooked by it. I'll try a few hours a day until he gets used to it or I can send him back. The neck was fine with plenty of room around the leg holes. I really wish this would work so he can stop wearing his cone after his bladder surgery. UPDATE: By the next morning he was fully accustomed to it and acting normal. No problem if he goes to the toilet or soils the material. He could eat and drink - so much better than a bag. It's wider than it's longer, so I twisted the leg straps a few times so he couldn't get out of them (which he did twice).
