Intermittent cable problem. First it worked, then it didn't work, then it worked, then it didn't work.

Worked fine initially, but the video disappeared after using it 3 times. I reconnected and checked, it worked for a while and there was a hum in the audio (fixed when I tried a different cable). I assume there is an intermittent problem with this cable.
