As a bike mechanic I CAN make manual adjustments but it makes the job easier. Works as written if you use it correctly. I have a suggestion: if you are using this on a bike with tires, deflate just enough to allow the top edge of the tool to rest on the rim as desired. I'll bet more than half of those complaining put it on the rim wrong. The rest, complaining that it doesn't work, are just trying to adjust to get rid of the squeak. They either don't try or don't realize that there can be more than one reason for squealing brakes. Others involve something as simple as cleaning the braking surface AND the brake pads with alcohol on a rag or something. You'll be surprised how much dirt you get off the brake and pad surfaces, even though it looks good. Second, the surface of the brake pad could be smooth. Try sanding it down with 600 grit sandpaper. If they still squeak, replace the pads.
