I have ordered several of these toothbrushes. Each is marked "Medium". However, when I used one, I found that the bristles are for a SOFT toothbrush. FYI, even though they cost less than a dollar each, the seller charges almost TWO DOLLARS FOR EVERY SHIPPING! And that despite the fact that they are all sent in a heat-sealable plastic bag. There is one more thing. This toothbrush doesn't fit in the old fashioned ceramic tile toothbrush holder in my bathroom. While the pen might look flimsy in the picture, it's not flimsy enough to fit in the old-fashioned built-in holders that many of us have in our bathrooms. Bottom line: don't buy this if you expect the bristles to be anything but soft. Don't buy it if you think $2 each is an exorbitant shipping fee. Don't buy it if you expect it to fit in an old fashioned toothbrush holder. The body (handle) reaches a width of more than 1/2 inch in front of the neck. The neck of the pen tapers to about 1/4" wide and 1/8" thick 1.25" in front of the head. The head (bristle area) is 1.125" long and has 11 rows. Clusters of bristles in each row: 2 ,3,3,3,3,3,4,4,4,4 and 3 from handle to tip.
