The corners are rounded and the nylon insert is too tight
Those things are stupid tight! One of them got caught on a bolt. Even hand tools they screw onto a bolt heated everything to near-uncomfortable temperatures. It doesn't help that the corners of the nuts are rounder than they should be. remove them safely.
