Check the loose connecting rod for wear.

Rank nuts fit and work perfectly on my Sugino VT crankset on a vintage 1980's Nishiki Sport 10 speed crank. The left handle came loose and the nut fell off. The initial repair failed, but not this mother's fault. The crank came loose again almost immediately and I had to tighten the nut every 3 minutes during my ride until the new nut eventually fell off as well. Glad there were two! The left crank also had to be replaced, as the soft aluminum crank was "rubbed down" by the hardened steel of the bottom bracket. After that everything is fine again.
