You can adjust the diameter to suit your needs by removing part of the filling.

I've researched many lumbar pillows and most of them had two main disadvantages: 1. They were too thick (too large in diameter) so they put a lot of pressure on the back. 2. They were too high, apparently designed for people with very long torsos. The lumbar pillow must match the height of the lumbar curve to support only the lumbar region. The diameter required will vary depending on how soft and squishy the chair you are sitting on. Over 30 years ago a physical therapist advised me to make my own by rolling up bath towels and using large rubber bands to keep them from unwinding. I have a skinny one for the car as most car seats are much more supportive than couches and armchairs. I have two thicker ones that together provide support in my chair or on my couch. Unfortunately the covers on these loin rolls are worn out so I was looking for a decent replacement. First I tried a self-inflating lumbar pillow. He was too skinny and too tall. Most of the others I examined were both fat and tall. Maybe they are all intended for NBA players? I was attracted to this lumbar pillow because it wasn't that high and allowed me to open and remove some of the buckwheat hulls to adjust its diameter. After removing about 1/3 of the buckwheat I had the perfect chair pad. I saved leftover buckwheat in case I need lumbar support in a chair or couch that is softer or irregularly shaped. My only complaint is that I couldn't find a decent inflatable travel lumbar pillow that didn't have bad leak reviews.
