Keep the strands of wire clean and straight.
The solder inside melts at a higher temperature than the plastic coating. If the wires to be joined have a small loose strand, the soft plastic will easily penetrate that strand during the melting process, causing the bare wires to protrude from the sheath and defeating the purpose.
