This helmet is highly adaptable to many head shapes. It also covers the back of the head better than traditional bike helmets. We chose this helmet for both of these reasons, as our daughter has had multiple cranial surgeries, most recently on the back of the head, and we wanted to offer her the best possible protection (after a healing period directed by her surgeon). . She's close to the upper limit of the measurement for the size we're listing, but the area that tightens is under most of her head (not sure if this is her head shape or typical), so we need to do the tightening loosen duct tape every time we take it from time to time, but that's okay because we prefer to be safe.
