I bought this during a recent bout of sacroiliac pain. It's adjustable enough to fit my stomach with some weight fluctuations. It has a cut-out at the back to cover the sacrum as well as the lumbar region, which is important for those experiencing pain from L4 and below. The Velcro on the front is divided in two so that the ice pack does not twist or slip. My only complaint is that it doesn't stay cold very long, but the ice cream should only stay for 15-20 minutes at a time anyway. I also have a similar shoulder bag and it's great. It's worth the extra money if you have chronic problems.
