I wear shoes 7.5 and SMALL fits me correctly. The tongue is well padded but a separate piece. I use it in place of the ridiculously heavy and uncomfortable cast they gave me in the ER. We'll see what my podiatrist says, but I feel more stable and secure in them than in heavy, clunky and clunky 5 velcro knee length black boots.
