Came inflated, and not fully inflated which was perfect because it gives you the ability to rotate and shift your weight easier. I am quite impressed with the way it actually helps stretch your spine while sitting. I read thru many of the reviews which is what convinced me to buy one. I did have a sore back the first few times I used it, am guessing because of the stretching of the back, but the soreness was quickly gone. Just proves that it does shift the weight around and it definitely provides relief. Sometimes I put it under my feet to give them some relief. Definitely recommend!
