The micro USB ball plug is not fully inserted into the connector.

Unfortunately there is something wrong with the way the female Micro USB connector is made, it does not allow the connector to be fully inserted. It seems that this adapter and the cable that plugs into its micro jack will eventually just fall apart through the forces of nature. hoping for the best.
