It comes with an extender for an elastic band that fits over the back of the office chair for extra head support. The strap isn't adjustable with a buckle, but the elasticity of the elastic is enough to fit my chair. I put a screw in the hinge to tighten it a bit. I found the padding that came with it to be too thick and stiff, so I took it out and just put in some foam I cut out of the mattress topper.
