The most common failure mode I've found is when someone steps on the buckle while it's on the ground and not latched. After losing two straps this way, I got into the habit of snapping them in place every time I took the strap off. Eventually the plastic will fatigue and crack on its own. It doesn't bother me that much as it will last for many years. And since I haven't found a belt that compares to the Bison Designs belt, I'll keep using it until I find a better one.
