My podiatrist recommended I wear it in flat shoes without removing the insoles which can be removed. So far I've worn them in 3 very different flats for a full day each. They were very comfortable. I thought they might hurt because most shoes with arch support hurt. To my surprise, the arch support of these insoles is not painful. They don't slide forward either. One of the downsides is that they lift the foot a lot so I can't wear them in some shoes. They press my feet too hard on the top of such shoes. Okay, time for an update after a few weeks of wearing: they do slide forward at times. Not bad, not often, but sometimes I have to take my shoe off and put the orthotic back in place. These orthotics also work great in closed-back sandals, but the blue on top and off-white on the bottom show the sides, so I wish they came in plain brown or black.
