Small means small on my review.
Bought the wrong size at first, the small one would maybe work for a guinea pig or kitten. Regular size cat should probably go for a medium. I have two smaller cats so I went ahead and kept the small cone too. It worked well for my boy to keep him away from his eye. It was easy to put on and take off.
