Love this case! I use it when I have to walk or stand for a few hours or more. I've tried a couple of neoprene sleeves that offered good grip but weren't very comfortable. This case is very comfortable. It doesn't offer the same level of support or tightness as neoprene sleeves. The neoprene sleeves I tried were very tight and uncomfortable. And yes, they are true to size. It felt like they were too tight and were causing the muscles to move in weird directions. This case is perfect for my needs. It's like a good warm sock. This keeps my stiff knee warm and more flexible. There is good support but I can still bend my knee. It helps me stand longer and doesn't bind like others. He stays on his feet without hitting my leg. My knee problem is in my knee and it's completely painless because there are two pieces of nylon bone or something in the back. It doesn't "grab" my kneecap. I don't know about vital silver - maybe it will work - but the case is just great, I'm considering getting another one.
