You can do it yourself. Pretty painless and easy. It comes with no instructions, but if you search YouTube for ear piercing, you'll find videos showing you how. There is no marker included but there are 2 small alcohol wipes. The main task for me was to make sure that the back is in the right place. I placed my ear against the back of the device where the plug is located and slowly pushed down to align the earring's shank with where I had marked it. Then I pressed hard very quickly and the cello is finished. The first was painless and perfect. The second one was ok but for some reason it didn't make it to the end. but it was easy to poke the rest of the way and still painless. I would buy again and recommend. I bought 3mm and they were the perfect size. ****Update in 2 weeks**** Ears are healing properly. The earrings have a great shape and are very comfortable. Great price and great purchase.
