T-shirt fits perfectly. I bought the 4T for my slim four year old. You want to go taller if you have a bigger child or know their head is slightly larger than average. My poor child can hardly get it in the hole. I had to stretch it as far as I could with both hands and then do it as fast as I could. She didn't like it. But when it's on, it doesn't fit that well. If it was part of her wardrobe I wouldn't keep it to myself. But it's perfect for the DIY Halloween costume I'm making for her.
