I ordered Large/Large because the one I already have is Small/Medium and too tight for my hand. I have the large/x-large and it fits my wrist even more snugly than the small/medium I already had. The hand piece isn't any bigger either, it's just a hole for your index finger and a big hole for 3 fingers. I'm guessing large/x-large means a bigger hole for your 3 fingers.
