I have never used loose tea because it looks so dirty. The diffuser works great. Simply push the plunger to separate the two halves and extract what you need. Let it sit back and shrug off anything that might be sticking out of the rim. I use it with my Keurig and just run hot water into a diffuser cup. It's not ideal. Depending on the type of tea used, very small particles settle at the bottom of the cup. Particles are way better than whole sheets and pieces of other things. If you want to pour it all out you'll need some sort of strainer and a second cup, but you can just use a diffuser and leave a sip at the bottom of the cup.
