The pillow came with a hole in the cover. Doesn't affect use in any way, but it's frustrating when you're ordering something new that hasn't been tested. I wanted a car seat cushion and didn't want the hassle or waiting for a new one. So I just did it myself.

I found this while looking for something to help with a flat spot on the back of my child's head. He seems to love it. He is 2 months old and it really helped round out the area.
