Fell off randomly 3x

The personalization is great.The collar has randomly fell off 3x. Im not sure why or how. First time he was being petted and it was found on the bed. Second time the dog was standing in the livingroom and I saw it on the floor. Third time, in his crate and he had chewed/broke his samsung smart tag (so I had to buy and spend money on another).
