The base does need shaping and the ends filed to either sharpen or just smooth rough edges. The nail doesn't have a natural bend; they are very straight out. I am 5'6" and 150 lbs, usually a solid medium in most everything. I ordered a Large (per other reviews which said runs small) which fit really nicely, but if you like a super tight fit I think a medium would actually have worked. My cat LOVED this toy. His favorite thing to chew on. He would hug it and chew it and his eyes would roll back because he was so happy.
