Why doesn't it stay in place?

Update: I've had it for about 8-10 months or so. The TV has never been dropped BUT it has never stood up straight. I installed it professionally on my 50 inch plasma tv which is well within the weight limit. My TV doesn't look straight. It always rotates slowly randomly. It's pretty annoying. , the stand is EXTREMELY tall (but it was pretty much the only option I could find with high weight limits to hold the plasma). She is VERY shaky. I'm waiting for it to just tip over and smash my TV. Try to find other options.
