Arduino Mega doesn't always drive it active low or active high

I needed a 12 volt driver, not a 5 volt driver, this seemed like a good replacement (red in photo) 5 volt board (blue in the photo), but the same Arduino sketch does not enable this board. All 4 relays are activated when Arduino is connected. To be clear, nothing happens without an input, and choosing Active High or Active Low works on the bench. However, once the Arduino is connected regardless of the input selector jumper. Relays switch on.
