Fits my 2004.5 Chevy Silverado 2500HD LS Crew Cab with dual climate controls.

Suitable for my Chevy Silverado 2500HD LS Crew Cab 2004.5, with double air conditioning. Fixed my issue where only the bottom four speeds were working. (tried a resistor/relay first as that must be a common cause of this problem).
