The mounting screw protrudes enough to prevent the shaft from fully submerging in the clamp and is held securely. Also, the spring action is a bit weak. It can happen with heavy objects make sure these fall out of the clamp Make the clamp deeper so that the set screw does not interfere when the clamp fully grips the shaft.
