The best place to look for a defect is under Accessories & Consumables.

The electrical contacts for "hot" and "neutral" at the end of the socket were held in place solely by the spring tension of the contacts themselves and their friction against the top half of the clam shell. They are about 0.015 to 0.020 inches above the grounding lug, which is firmly attached to the bottom shell of the clam with a set screw. There was no insulating material between the hot or neutral contacts and the ground lug. In the absence of an insulator, there are two conditions under which the hot and/or neutral contact of an outlet can move and contact the ground lug, causing an AC short. The first uses the "VOLT CHECK" connectors by inserting a voltmeter probe into the connector. The probe presses the contacts into the grounding lug, causing a short in the live AC line. The second reason is that the contacts can be disengaged by gently tapping on the lower half of the housing near the end of the socket. This can happen if the device has ever been dropped. This in turn will cause a short circuit in the live AC line. Speaking to a manufacturer representative, I discovered that there should be an insulator between the hot and neutral terminal and the ground terminal. If you have one of these, make sure the insulator is in yours. UPDATE: The manufacturer replaced the defective product without question. The replacement has a well-designed insulator.
