There were dust particles on the sticky side of the screen
I completely cleaned the screen as described. I made sure there was nothing on the glass. I noticed a few particles on the sticky side of the screen but couldn't remove them. After application, the defects formed bubbles that could not be corrected. It's not aligned properly either - you have to cover the camera lens slightly. There were dust particles on the sticky side of the screen.
