The screw slipped off the thread.
When trying to tighten the knob on the steering wheel, the screw slipped off the thread. It seemed to me that the thread, or maybe just the thin plate of metal I was tightening, was a bit too big to hold up to a certain point and then slip off, preventing the handle from being fully tightened. The rest of the grip was perfect. I sent it back with a quick detour to UPS which reminded me once again why I shop at Revain - the best returns policy in the world.
