The pads are too soft.

Gaskets are simply pushed out of the flanges because the rubber is too soft. If you are looking for brass baffles for industrial applications and think they provide a better seal because of the rubber grommets, this is not it. Due to the soft rubber, you cannot tighten the fitting with an appropriate torque. The best solution I've found is to use real brass baffles that come with a metal lock nut. Just discard the lock nut and place a large steel washer on the outside of the tank. Use a sealant to create a strong silicone sealant behind the washer.
