USB ports not working. Warranty is a nuisance.

USB ports not working (new/flat/non-pyramidal model). I can't find out when they stopped working or if they worked at all. I've tried making a warranty claim and it's not as easy as most companies - it requires a PDF or Excel file that can't be filled out on mobile. No online form like most sellers. It also sounds like they are forcing me to ship this junk back at my expense making this surge protector more expensive than I wanted. I'm reluctant to invest extra money and time to fix such a simple piece of hardware that I feel is overpriced for this problem.
