Must be held face down to allow eye to rest. I have a covert credit system on my desk, I bought this thick foam pillow to sleep in a prone position in bed. I wedged a foam pad between the large pillows to keep it from moving. That works well enough. Then I accidentally turned the pillow sideways so the opening faced the shielded eye and found that it supported my forehead, cheek and jaw better, with a much larger open area so the shielded eye couldn't touch anything! The height is perfect for me to keep my head parallel to the floor with comfortable support compared to the "professional face down system" which has a metal bar that always rests on my jaw no matter how I position the headrest set I have removed this system and only use this foam pad when I need to rest face down.
