First of all, Plugable has been very helpful and has always responded promptly to my support requests. Special thanks to David for his help! I bought this adapter to use my Samsung 4k TV as a secondary monitor for my 2015 Macbook Pro with Retina via the TV's HDMI 2.0 input. Initially the installation seemed to go smoothly as once plugged in I was given the option to output 3840x2160 in the display settings with the option to output at 60Hz. I didn't need to do any additional settings to get this option (note: holding OPTION + clicking zoom brings additional resolution and refresh rate options), although I've read articles to the contrary. However, on the first try, outputting at 60Hz only resulted in garbled clutter on my display. To fix this, after much frustrating trial and error, I used a third-party software called "SwitchResX" to generate a "custom" resolution via SwitchResX. Instead of using the various clock settings/settings on my TV, I went with the default "CVT-RB" profile. After saving the custom resolution to make it available (and rebooting), the TV now correctly accepts the 4k@60hz output from the Macbook through the adapter and displays it fine. My advice to anyone having trouble with 4k@60hz and an Apple computer: first make sure your hardware is capable of outputting 4k@60hz, then use SwitchResX to add a CVT-RB profile , if you're having trouble with the resolution settings that OS X natively provides. It was not immediately apparent that the fine-tuning of the output resolution. Hope this helps someone else who encounters the same problem.
