It worked for a while - then I had to operate

Everything worked fine for several weeks, and then I noticed that one side wasn't turning. I tapped the back a couple of times and noticed the blades got stuck in the back grille on one side. Upon further inspection (see images for details) the nut on the bolt holding the fan blade came loose. The blade then protruded just far enough to catch on the back of the device. Additionally, the hole that the bolt goes through is a semi-circle designed to line up with the bolt itself to prevent rotation when engaged. The blades must have hit the back of the grate hard enough to make it rotate as it will no longer align properly on the bolt. I figured I could fix this by simply cutting a small square off the grille on the back to access the studs, but I couldn't get the blades high enough while the unit was still assembled. I probably should have just skipped this attempt and taken this thing apart from the start, but the way the flaps come out seemed like a pain to me. In the end I was able to separate the front from the back and had free access to the fan blade and associated bolt. I adjusted the blade on the bolt, lubricated it with Loctite and reinstalled the bolt. It now works as intended again, but I suspect the screw was loosened at the factory. I can't think of any other reason why it might kick off. The bolt on the other blade was stuck so I didn't bother with it.
