Solved my problem with the 5-coupling
This thing saved me. I was determined to get an Andersen universal ball hitch for my 5th bike. I have factory discs on my 2012 f250 frame and the previous owner removed the pivot ball. So I bought it to attach to shims and then attached Ultimate Steel Andersen for rails. This made it sit higher than I would have liked, but I am very happy with the product. One of the holes didn't quite fit so I had to file it down, but other than that it's easy to assemble and install (and I'm a single, middle-aged chick so not really Mr. Fixit).
