I would go down a size or wear a normal size.
I used to wear medium before pregnancy, sometimes small (junior) depending on how it fits (125 pounds). After reading the reviews I chose the Small because everyone said it needed more compression in the abdominal area and I think I made the right choice. I'm 5ft 4in and 151lbs after birth. It just said to leave the tags if you want to return them but they were so tight it took some effort to pull them out that I ended up tearing off the tags. They were long enough for my long torso when I lifted them all the way up. I don't see any underwear lines.
