Medium fits my Thoroughbred 16 hrs, 1000 lbs. I bought it because one of his blankets was rubbing his mane and causing pain in the withers. I liked this one because it seemed like it was going higher on the neck which helped with his mane. However, it doesn't fit snugly around the neck to stay upright. And the rubber band that goes around his girth pinches his back, it left a sore spot just below the withers. But the quality of this shoulder pad is good
