I got the medium for my 7lb Shih Tzu and it seems to fit. I had to pull it a little tighter than the velcro was supposed to, but it should stay in place. It doesn't seem to bother her, she hasn't tried to remove it yet. She has a spider bite under her left ear to prevent scratching it. Seems really durable. I don't like the sticker, you can't just peel it off. After dealing with it, I gave up and basically left it on. I also put an "inside out" cone on it. The barbed part of the velcro was supposed to be on the inside and I didn't like that. It seemed awkward so I turned it over so it was facing out.
