We have a new baby Chihuahua who is 5 months old - small and only 4 pounds. Her previous harness fit "up to a 10" so I bought her this X-Small based on the marked measurements. First, the head hole is so tiny. and our pup actually has such a tiny head for her body. She squeaked even though we were very cautious trying to get it off her :( And the straps were loosened to the max and it barely reached her girth. So bottom line I'd definitely UP THE SIZE which you think you need. Otherwise a lot well made, just a little difficult to put on - even if it fits.
