I used to have a knife: NRS Co-Pilot Knife Yellow One Size. Both knives are similar in that they are both sharp and blunt to avoid injury. Also, both knives were designed to be easily removed from their sheaths, which is where the problem lies. The sheaths of both knives attach to the loops of your life jacket. Both fit snugly to ensure the scabbard does not come loose. If you perform any type of rolling or self-rescue where you can even slightly bump into the knife/sheath, you could potentially lose your knife. I have had the same experience with both knives. Today I rode and did a cowboy self rescue and halfway through I realized my knife was missing. So disappointing. The knife itself was great. It stayed sharp after cutting a lot of rope while traveling, but as I wrote in my review of the Pilot knife, if you can't count on your knife to stay locked, it's not worth owning.
