This item is used with the PetSafe Solvit Deluxe Telescopic Ramp to stabilize the ramp using the car's side door frame as an attachment point. While not strictly necessary to use, as my dog proved to me climbing the ramp while I have the harness attached to it, it gives the ramp stability to stay in place and ensures that there are no accidents when the ramp slips off the seat and let your dog get in/out of the car on the ramp. When you get a ramp, you pay money to get a harness. It's worth it.
